Reporting Procedure

Alcohol Counseling Reports

Note that any proof of attendance which includes your full name, the County Court at Law No. 3 cause number and the information required below on your report is acceptable. This form is for your convenience.

Your report must include the date, time and alcohol counseling you attended (for example "AA - Glass House", "AA - Triangle Group" or "Susie Smith, LSW"). Each session you attend must be verified by the signature of the meeting leader, Chairperson or counselor.

Failure to timely report required counseling will result in the immediate revokation of your Occupational Driver's License without further notice. This revocation will result in a 60-day period during which you may not receive a license from any Court.
