COVID-19 Vaccination Records
Due to the unique nature of the COVID-19 pandemic and vaccination requirements, Tarrant County Public Health (TCPH) offers its clients and all residents several options for replacing lost individual vaccination records.
Option 1:
The quickest way to get replacement COVID-19 vaccination record cards
Come in person to Tarrant County Public Health's Vaccine Distribution Center, located in TCPH's main campus building at 1101 S. Main St. in Fort Worth. The center can provide copies of immunization records, and replacement COVID-19 vaccination records on a walk-in basis.
The Vaccine Distribution Center is open Monday - Friday from 8 - 11:45 a.m., and 1 - 4:45 p.m.
Regular immunization records cost $9.
Replacement COVID-19 CDC vaccine records are free.
Option 2:
TCPH client vaccination records
If you received your COVID-19 vaccination at a Tarrant County Public Health clinic,* you can access the vaccination record by logging into this portal.

Records from TCPH COVID-19 vaccine-only vaccination sites
If you received your COVID-19 vaccination at one of the Tarrant County COVID-19 vaccination sites listed below** between Dec. 21, 2020 and July 14, 2021 you can access the vaccination record by logging into this TCPH portal.

COVID-19 records from other health care providers
If you were vaccinated by another health care provider and require an official ImmTrac2 record, click here.
If you don’t remember who administered your COVID-19 vaccine or if you received a COVID-19 vaccine from more than one provider, select this option and follow the instructions.
Frequently asked questions regarding COVID-19 shot records:
1. What is an official immunization record?
An official immunization record is provided in Texas by ImmTrac2. ImmTrac2 is the Texas Immunization Registry service offered by the Texas Department of State Health Services. This secure registry safely consolidates and stores confidential immunization records from multiple sources onto one centralized system. This system supplies local health departments with data needed to generate health records.
2. How will my vaccine record be delivered?
When applying for a vaccination record, you have the option to pick it up in person, download and print to PDF, have it mailed to you, or sent to you by an encrypted email.
3. What if my COVID-19 vaccination record was not found?
If we are unable to find your record, we will notify you within five business days following your request. You will then need to contact the provider who administered your COVID-19 vaccine.
4. If I noticed a discrepancy on my record, who can I contact?
Please contact the provider who administered the COVID-19 vaccine you received.
